IN-STUDIO SCREENING FORM

In an effort to reduce the risk of coronavirus exposure to Rick Imus Music Studio staff and other students, all students must allow their temperature to be taken and complete the following screening questions:

  1. Have you travelled outside of Canada or been in close contact or with anyone who has been to a country listed on a Travel Advisory by the World Health Organization or Public Health Agency of Canada?
  2. Have you had close contact with or cared for someone diagnosed with coronavirus in the last 14 days?
  3. Have you experienced any cold- or flu-like symptoms in the last 14 days, e.g., shortness of breath, fever, cough, loss of taste or smell, difficulty breathing, sore throat, headache, muscle pain? If so, describe your symptoms more fully here:

 

Students with temperatures higher that 37.78 degrees Celsius or who answer Yes to any of the above questions, may have their appointment rescheduled or re-assigned to the online format.
